What is Medical Male Circumcision?

Medical Male Circumcision is the process of removing the foreskin, thereby completely exposing the head of the penis. There are many benefits to adult circumcision. After a series of medical trials, it was observed that circumcised men are less prone to HIV from their female partners. At the same time, World Health Organization has endorsed adding circumcision to HIV risk-reduction intervention packages.
As the inner skin of foreskin is more prone to minor tears during intercourse, this part is more likely to absorb HIV virus than any other skin. It is the Langerhans and other receptor cells within the foreskin that is responsible to attract HIV virus. Also, uncircumcised men are more prone to other sexually transmitted infections like Balanitis and Chancroid.  Presence of these infections increases the risk of HIV infections in men.

  • Reduces the difficulties that older men suffering from prostate gland and bladder problems usually face
  • Offers a large degree of protection from cancer of the penis
  • Reduces cancer of cervix on women because of Human Papilloma Virus.  This virus thrives under the foreskin of men, which gets transmitted to women during intercourse. 20% of such cancers can be avoided if men are circumcised
  • A circumcised men is 8 times less prone to HIV virus than uncircumcised men
  • Circumcision in men can prevent diseases that cause inflammation like Balanitis
  • Lower risk of other sexually transmitted diseases

A Big Leap in the Field of Infant Circumcision

It is true! The new method brought in by Dr. Neil Pollock, a Vancouver doctor was really a revolution. The Vancouver Sun published an article on Dr. Pollock and the new circumcision technique that he has come up with. An article ‘New circumcision technique saves time, resources’, published in ‘The Vancouver Sun’ on November 16, 2012 clearly states about the new technique and Dr. Pollock’s opinion as well. This article written by Mike Hager covers different aspects of the infant circumcision techniques of Dr. Pollock, even with the benefits and opinions that experts share about circumcision.

In the new circumcision technique developed by Dr. Pollock, glue is made use of instead of sutures to close the wound. This technique is mainly meant for patients aged above one year. In this process, the patient will be treated under local anesthetic. This new technique was done for the first time to treat 14 month old son of Flynn and Flora Yu, who was thrice affected by Urinary Tract Infection within the period of nine months. “For us it was a medical issue so we didn’t really have much choice,” said Mr Yu, father of the 14 month old boy.

When it comes to the process, Dr. Pollock clearly explains it is a simple procedure wherein the patient would be given local anesthesia and glue would be made use of to quickly close the wound. Earlier, patients undergoing circumcision were given general anesthesia and sutures were made use of to close the wound. This was a long process done in the operating room of the hospital. Stitches or glue isn’t necessary if the boy is under the age of one as the skin can heal naturally. Dr Pollock said, “I can do the whole surgery and enclosure almost faster than they can roll them in and out of the operating room”. “The benefit to the system is we can off-load all these kids.”

The opponents of infant circumcision argue on this process stating that at such a small age, children don’t have the capability to express their consent to the process. In addition, they also argue that the process reduces their sexual satisfaction in future. On the other hand, in August 2012 the American Academy of Pediatrics came out with new guidelines basing on the review of over a thousand articles. The guidelines were then published in journal Pediatrics which endorsed infant circumcision. Yet, it wasn’t ultimately in favour of the infant circumcision process as it fell short of recommending universally. It stated that the final call is all left to the parents to decide whether or not to have their infant circumcised.

The circumcision process was justified by scientific evidence which suggested a reduction in the risk of Urinary Tract Infection. Apart from that, circumcision even reduces the chance of penile cancer and sexually transmitted diseases like HPV and HIV.

The Canadian society which isn’t supportive to routine circumcision for infants, is now reviewing on the controversial subject. The society will be publishing a study “written by Canadians for Canadians to help parents make that decision,” within a year.

As the medical professionals in Canada haven’t supported in favour of infant circumcision, the British Columbia’s Ministry of Health pays only for the circumcision which they consider as medically necessary. Cindy MacDougall, the spokeswoman of the ministry told The Sun that between 2011 and 2012, 1153 medically necessary cases were reported, costing $174,670 to the province. In the year before, the province had to cover the expense of 1,387 circumcisions.

Infants suffering from a foreskin infection like phimosis, paraphimosis, posthitis
or Urinary Tract Infections are the ones who need to undergo circumcision upon any doubt. When it comes to boys and men, circumcision helps them to get over infections like HPV and recurrent balanitis. The average cost of the circumcision process is less than $150 per surgery. The cost would be a bit high in the case of infant circumcision. For babies of 2 months or younger, Dr. Pollock charges $445 for the circumcision process. The charges for more medically necessary cases referred by pediatricians are to be figured out yet. With different American establishments supporting the infant circumcision, across the world Dr. Pollock has observed a huge leap in the number of parents requesting this process for their children.

Apart from Vancouver, Pollock extends his services to different cities through expert doctors trained under him. Dr. Buenafe is one among them who is well experienced in undertaking the novel Infant Circumcision procedures developed by Dr. Pollock. Virtually painless, quick (30 second procedure) is one such procedure which has been performed successfully on more than 35,000 circumcisions.

Benefits of Infant Circumcision

After the birth of a son, the majority of the parent population are worried and some consider undertaking this procedure on their son.. Circumcision is theprocess of removal of foreskin from the genital part of a malebaby. This is quite a common process performed on newborns, particularly in the United States. Parents usually find it hard to make a decision whether to get the process done or not. Earlier, it was a cultural and religious custom to get the circumcision done. Hence, there wasn’t much issue with respect to a decision to be made. Later as time passed by, infant circumcision has become the sole responsibility and decision of the parent.

Parents usually step back from this process worried about the pain and risks that may come along with. In such a scenario, the members and other stakeholders of the American Academy of Paediatrics (AAP)grouped up to form a multidisciplinary task force in the year 2007. Their main aim was to update the recommendations made by the Academy in the year 1999. The result of the evaluation done on the recent evidence highlighted the fact that benefits of infant circumcision outweigh all the risks involved in the process. In addition, it justifies the decision made by the families that supported the process. The Circumcision Policy Statement of Pediatrics Volume 130, Number 3, September 2012 clearly explains about this fact. There are numerous benefits with respect to this process. Few among them are as given below.

Urinary tract infection
UTIs are not much common in men. Yet, infants below the age of one have a high risk of this infection. Studies have reported that when compared to circumcised male infants, uncircumcised infants are more susceptible to this infection. If overlooked, it may even lead to kidney infection as well. Circumcised infants don’t need to worry about this infection.

Penile Cancer
Circumcised men don’t need to worry about getting affected by penile cancer. Whereas for uncircumcised men is a matter of concern. At the same time, women with uncircumcised male partners is more susceptible to cervical cancer as well.

Sexually transmitted infection
Circumcision reduces the risk of sexually transmitted diseases like HPV, Herpes Simplex Virus type 2 and HIV. The chances of these infections are really high in uncircumcised men. Though circumcision cannot eliminate the risk of these infections, it can reduce the chances to a great extent. Physicians usually recommend infant circumcision rather circumcising in later stages of life.

It is very common for parents to get tensed about the risks in the infant circumcision process. There are no risks involved if the process is conducted in sterile condition by expert and trained professionals.

Complications are very minor and rare. Infant circumcision is recommended as the complication rate will be considerably low when compared to later stages. Parents are responsible to take ultimate decisions. Hence, clinicians are strictly recommended to explain to parents about the benefits and risks involved with any bias. According to American Academy of Paediatrics (AAP), even the American College of Obstetricians and Gynaecologists have certified these facts.

Infant Circumcision – Weighing Between ‘Yes’ and ‘No’

With different opinions on infant circumcision, taking a right decision would seem to be a tough task. Making a decision just on the basis of word of mouth from your neighbours, friends or relatives would be unwise. It is necessary to know what the experts say in order to be confident in your choice. In recent times, there has been a great decline in the number of the infants being circumcised. Charge Data Master points a decline from 58.4 % in the year 2001 to 54.7 % in 2010. However, these figures do not include the circumcisions being performed outside hospitals. People getting circumcision done for religious reasons are the ones who prefer to get it done outside hospitals.

Antony Losquadro, the executive director of Intaction.org
expresses his opinion that parents need to realize that infants are perfectly intact by birth. “Although the reasons have tried to be rationalized over the decades as a necessary procedure, I think parents are becoming more educated on the issue”, he added.

Dr. Andrew Freedman, an associate of the American Academy of Pediatrics (AAP) task force on circumcision has pointed out that there has been a variation in the demographics as parents and certain ethnicities have started considering it as an unnecessary task. Yet, there are people who consider the positive aspects of this circumcision process and prefer to get their infants circumcised.
Few studies during the 70s and 80s show that infants pass through some pain during and after the process. Though anaesthetics are used, they don’t remove pain completely. Nonetheless, there are several pain relief methods that show effective results. Yet, many doctors do not use the anaesthetic since they are either not familiar with its usage or they are worried about the side effects that it may cause to the baby. According to Freedman, “There’s no question that these pain control techniques reduce the pain, and there’s no viable excuse not to use some form of pain relief.” Hence, parents willing to get their infant circumcised don’t need to worry about the pain aspect. Freedman also added that most of the reputed hospitals do follow policies that instruct to use minimum pain control standards.

Now are you worried about the risks involved in the process? Just like every other medical process, there are few risks with respect to circumcision as well. The clamps used in this process are usually large. Hence, it becomes tough for the physicians to remove the foreskin appropriately. Losquadro says, “It’s really guess work on the part of the doctor as to how much skin to remove.” It can be considered as a risk factor. The process has few other risks associated with it as well. Curvature of the penis, bleeding, scarring, infection and skin adhesions are a few to point out. Yet, risks like these are extremely rare. A report in the BMC Urology journal clearly states that frequency of complications is just 2 percent. On the other hand, there are numerous benefits that outweigh the risks of infant circumcision.

The revised policy statement issued by the American Academy of Pediatrics in August 2012 clearly outlines the benefits related to the infant circumcision. With the first year of their life, the chances of Urinary Tract Infection are really high in infants. When it comes to uncircumcised infants, this risk is really low. The studies conducted by the American Academy of Pediatrics show that the risk is 1 out of 1000 in circumcised boys and 1 out of 100 in the uncircumcised group.

In addition, infant circumcision even reduces the possibility of penile cancer and protects from infections that are transmitted sexually. Another study conducted by the American Academy of Pediatrics points out that the chances of Herpes Simplex Virus type 2 is 28% to 34% lower in circumcised men. The possibility of contracting HPV has also decreased by 30 to 40% in circumcised. It is same the case of contracting HIV as well. The World Health Organization reports show that the chances for man to get infected by a HIV partner is approximately 60% less if he is circumcised.

Even the chance of foreskin infection is lower in circumcised men. “On balance, we feel that the benefits probably outweigh the risks, but not by such a margin that we’re advocating circumcision for everybody,” Freedmand stated with respect to the process. The medical aspects are never going to hinder the process of circumcision as the studies show that the benefits really outweigh the risk factors. Yet, it is the responsibility of the parent to decide on what is good for the baby. “In the end, whether you decide to have your baby circumcised or not, it’s a personal decision that you should feel confident making,” says Julie Revelant in her article ‘Circumcision on the decline?
